ACH Request Form

This form is used to request to receive an ACH Authorization form to sent to you via DocuSign.  All information sent via DocuSign is sent encrypted.  Please do not send any banking information via email.  Please allow 24 hours to receive an email from DocuSign.  Most requests are processed the same day it was requested.  If you prefer to receive a paper check please do not submit this form.  Please note that due to our payment processor checks can take up to 14 days from the date MAGDRL sends the payment instructions to the processor.  Unfortunately this is out or our control.

The Mid-Atlantic Great Dane Rescue League, Inc. (MAGDRL) was organized in 1979 to serve the seven states of the Mid-Atlantic region. The organization is a DC corporation and has a charitable tax-exemption under 501(c)3 of the Internal Revenue Code.
